ELIGIBILITY: OPEN TO NATURAL PERSONS WHO ARE RESIDENTS OF THE 50 UNITED STATES AND 18 years of age (or the age of majority in your state, if different) OR OLDER AS OF THE DATE OF ENTRY

PRIZE(s): There thirty-five n) prize winners for this Contest. Participant acknowledges that the prize will be provided to its Nominee and not to Participant. The First Prize winner will receive $5000 (US) and two packages of insoles per year for ten years. The Second Prize winner will receive $2500 (US) and two packages of insoles per year for five years. The Third Prize winner will receive $1000 (US) and two packages of insoles per year for two years. Seven finalists will each receive a $200 gift certificate (US) and two pages of insoles. Twenty five Participants will each receive two packages of insoles.

Total Estimated Retail Value of the First Prize combined is $5,300 (US) which is comprised of a $5,000 (US) cash payment in the form of a check from Sponsor to the winning Participant's nominee, and twenty pairs of PAM insoles for the winning Participant and its friends and family (value of insoles provided not to exceed a total ERV of $300). Total ERV of the Second Prize combined is $2,650 (US) which is comprised of a $2,500 (US) cash payment in the form of a check from Sponsor to the winning Participant's nominee, and ten pairs of PAM insoles for the winning Participant and its friends and family (value of insoles provided not to exceed a total ERV of $150). Total ERV of the Third Prize combined is $1,060 (US) which is comprised of a $1,000 (US) cash payment in the form of a check from Sponsor to the winning Participant's nominee, and four pairs of PAM insoles for the winning Participant and its friends and family (value of insoles provided not to exceed a total ERV of $120). Total ERV of each prize for each of the seven finalists is $260 (US) which is comprised of a $200 (US) cash in the form of a gift card from Sponsor to the winning Participant's nominee, and two pairs of PAM insoles for the winning Participant and its friends and family (value of insoles provided not to exceed a total ERV of $60). Total ERV of all prizes for each of the twenty five participants selected is $1,500 (US) which is comprised of two pairs of PAM insoles for each winning Participant and its friends and family (value of insoles provided not to exceed a total ERV of $60).
