Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY), and must indicate which Prize Pool they would like to enter, on a 3½"x5" card and mail it (in an outer mailing envelope) with sufficient postage to Biofreeze For Life Instant Win and Sweepstakes, PO Box 251328, West Bloomfield, MI 48325. A Mail-In Entry is worth one entry. An entrant may mail-in one per day during the Promotion Period. Following this method of entry, an entrant will receive one entry into the Sweepstakes.
