ONE GRAND PRIZE: $5,000, payable in the form of a check.

Approximate Retail Value : $5,000.00.

SEVEN WEEKLY PRIZES (one awarded per Weekly Entry Period): $500, payable in the form of a check. ARV: $500.00.

Odds of winning a Sweepstakes Prize depend on the number of eligible entries received during the applicable Entry Period. Limit: One Sweepstakes Prize per person during the Promotion Period.

INSTANT WIN PRIZES

FIFTEEN FIRST PRIZES: A $100 Amazon.com Gift Card.

FIFTY SECOND PRIZES: A $50 Amazon.com Gift Card.

Where are players supposed to look to see if they're a winner for big bucks?

What is going on? And the last 8 months I've been notified five times that I've won big money over $5,000. This is not one of those you could win or you have not won yet, this actually states that I've won $5,000 or more depending on which notification I got. I've been told I would need to fill out a verification form that they would send either by email or direct mail and I have not received anything yet. So we go back to my original question what is going on?
